We strive to create safe, healthy, and inclusive communities for all.Role PurposeThis position will provide outreach support to client(s) referred to Connective by Community Living British Columbia to enable their adjustment to, and success with life in the community.Status and ClassificationUnionized. to provide the assessment that will ascertain needs and postulate intervention possibilitiesSupports individuals in their housing/residential settingsWorks with the client to consider issues and creates a mutually agreed intervention and support plan that has measurable short and long‑term goalsEnsures issues of daily living, including but not limited to housing, nutrition, clothing, hygiene, leisure lifestyle and access, employment and education are addressed in this planEnsures clients’ ongoing legal, mental, and physical health issues are addressed in this planWhen and as needed provides a wide range of interventions to enable the client to address and accomplish the established goalsInteracts with other professionals, family members, and organizations to ensure services, admissions, and treatments are in the client’s interest and supportive of the long‑term goalsOn the clients’ behalf, is a resource to other professionals, including but not limited to the courts, probation services, and community and institutional psychiatric servicesMaintains extensive records and case notes in accordance with CLBC policies and standardsCompletes monthly progress reportsParticipates in staff and tenant/staff meetings as instructed by the Program ManagerOther duties as subsequently negotiatedWorking ConditionsFunctions independently, while remaining a crucial member of the care teamAdheres to Connective policies and proceduresIs available to work flexible hours as needed, which may include some evenings/weekends, as determined by program/client needs and schedulesIs a member of CUPE Local 1936‑11Receives benefits and works within the working conditions as outlined in the collective agreement and personnel manualExperience and SkillsGraduate with a Diploma or Certificate in Social Services and one‑year of relevant experience is preferredExperience with clients with acquired brain injuries,



a history of conflict with the law, developmental disabilities and/or spectrum disordersInteraction with social agencies and experiencing psychiatric/personality disordersA demonstrated and clear ability to respond well in crisis situationsUnderstanding of providing service to people with different learning abilitiesExtensive knowledge of community services as they relate to persons with psychiatric involvementExperience in assessment, case/care plan development and implementationDemonstrated ability to deal with potential violenceDemonstrated ability to develop programs tailored to the particular diverse needs of the individualProfessional or lived experience working with vulnerable populations experiencing multiple barriersExcellent written and verbal communication skills in EnglishRequirementsAbility to successfully pass a reference checkAbility to successfully complete a Vulnerable Persons Criminal Records CheckPersonal transportation that is in good repair, insured for business up to $1,000,000 for liability, and which can be used to transport clientsProof of vaccination for communicable diseases may be required as mandated by Government, Health Authority, and/or funder requirementsCompensationAs Connective is a unionized environment, the starting wage is $27.54 per hour.
