client-centered treatment plansDeliver primary care services with a focus on preventive care and long-term health maintenanceParticipate actively within an inter-professional model of careProvide patient and family education on treatment administration, side effect management, and follow-up planningSupport community education sessions and participate in peer and chart audit reviewsOffer telephone support and act as a clinical resource for your healthcare teamMentor and precept NP students and medical students as opportunities ariseRequirementsEducationMaster’s-prepared Nurse Practitioner with a Primary Health Care certificateCertificationsCurrent registration in good standing with the College of Nurses of Ontario – Extended Class (NP-PHC or equivalent)Active NPAO membership with additional professional liability coverageExperienceMinimum 3 years of NP practice experience, including advanced nursing assessment in an inter-professional settingDemonstrated experience serving marginalized populations living with chronic illness, mental health challenges, and addictionDemonstrated ability to work independently with a high degree of self‑direction and strong organizational skillsPrior NP experience in a community health setting is strongly preferredPediatric experience is considered a strong assetTechnical SkillsAdvanced nursing assessment skills including physical examination and diagnostic reasoningEvidence-based treatment planning and clinical decision-making across acute and chronic conditionsProficiency with electronic medical records and community health documentation systemsCandidate FitThe ideal candidate is a mission-driven NP who thrives working in complex, community-based environments.
