Reporting to the Director of Care or Health & Wellness, your role centers on implementing and evaluating personalized care plans. You will be part of an inter-professional team, ensuring effective communication and coordination of care, while administering treatment and educating residents and families.
Key Responsibilities: • Implement and evaluate resident care/service plans • Administer medications and treatments following standards • Educate residents and families about care processes • Coordinate care assignments within the nursing team • Respond to emergencies and manage resident condition changes
Requirements: • Active LPN registration in applicable province • Preferred experience in senior care environments • Strong clinical knowledge and customer focus • Basic Life Support and First Aid certifications • Proficiency in computer applications for documentation
