As a Social Worker at SJHS, your central role will encompass both patient assessment and family counselling. You will provide evidence-based interventions and treatment modalities to meet the psychosocial needs of patients, ensuring a seamless transition from care to recovery. Key Responsibilities:
• Perform psychosocial assessments to identify patient needs
• Provide case management and coordinate necessary resources




• Facilitate crisis intervention and therapeutic counselling
• Document all interventions as per OCSWSSW guidelines
• Lead discharge planning with patient and family collaboration
Requirements:
• Bachelor’s degree from an accredited institution
• Registered with the Ontario College of Social Workers
• Preferred experience in an acute care setting
• Solid interpersonal communication skills
