The successful candidate will engage in individualized wellness plans, using evidence-based approaches to address the unique needs of clients facing mental health and addiction challenges. This position involves collaboration within a multidisciplinary team to ensure adherence to quality care standards while working towards client recovery and stability.

Key Responsibilities:



• Develop and implement individualized wellness plans for clients • Use assessment tools to improve client mental health • Provide trauma-informed and family-centered access to services • Maintain community resource networks and establish linkages • Ensure proper documentation and compliance with safety standards

Requirements: • Post-secondary education in Social Work, Psychology, or equivalent • Minimum 2 years in Mental Health case management • Knowledge of mental illnesses and experience in assessment • Solid risk management and crisis intervention skills • Valid driver’s license with access to an automobile
