Summary of Duties

To provide a full range of social work services to General Internal Medicine patients and their families who are admitted to medical units. Services include psychosocial assessments, counselling, discharge planning, linkage to community resources, facilitating Advance Care Planning Conversations and addressing complex care issues.

The successful candidate will participate as a key member of the patient care team on General Internal Medicine and various ICUs.

Qualifications/Skills
Master's Degree in Social Work
Member in good standing of the Ontario College of SW and SSW
Minimum 2 years of experience working as an MSW in a Hospital; acute medicine preferred
Experience with Gerontological patients an asset
