About MAiDHouse
MAiDHouse is the operating name of Assisted‑Dying Resource Centres Canada (ARCC), a national registered charity. We provide guidance, education, support and access to space for individuals eligible for Medical Assistance in Dying (MAiD) and those who walk alongside them filling the gap between healthcare and death care with knowledge, coordination, community, and dignity. We believe dying on your own terms should be accessible to everyone. All MAiDHouse services are offered at no charge, grounded in compassion and care.
Community Care Coordinator – Role Overview
The Community Care Coordinator is responsible for coordinating the delivery of MAiDHouse Toronto's on-site services. This role is built around a continuum of support from the moment someone first contacts MAiDHouse (whether through self‑referral or through an assessor or provider) through the day of provision and into the follow‑up care and resources offered in the days and weeks that follow. This continuum is grounded in guidance, education, and compassionate support, not clinical care.
MAiDHouse Toronto is staffed by two Community Care Coordinators, each engaged at 50% scope, working in equal collaboration on an alternating weekly rotation. The two roles are equal in standing and responsibility; neither holds seniority or supervisory authority over the other. Both
report directly to the Executive Director. This is a service coordination and relationship management position. The successful candidate will be highly organized, emotionally intelligent, adaptable, and passionate about ensuring equitable access to dignified end‑of‑life care.

How This Role Works
The two Community Care Coordinators alternate single‑week or two‑week rotations (structure confirmed with the Coordinator pair), Wednesday to Wednesday, so that each Contractor's rotation week includes the full weekend within it. During the rotation week, the Contractor is the primary on‑site service provider for all MAiD provisions, including those that fall on weekends. Weekend responsibility does not transfer between rotations. During non‑rotation weeks, the Contractor is available for clarification, consultation, and emergency backup coverage as needed. A standing Wednesday handoff check‑in between the two Coordinators supports continuity at each rotation change. MAiD provisions are typically arranged 48–72 hours in advance. The Contractor must be available to respond within that timeframe during their rotation week. The schedule may include early mornings, late afternoons, and weekends during rotation weeks. While each Coordinator is engaged at a 50% scope, the model is built for real life. The default is an equal rotation, but there is flexibility for the split to shift temporarily to 60/40 or 70/30 – to accommodate approved leave or changing operational needs. Any such adjustment is made collaboratively between the two Coordinators and with Executive Director approval. Base compensation reflects the contracted scope and does not change for minor coverage adjustments; extended coverage is addressed as set out in the contract. Time off is discussed collaboratively between the two Coordinators first, then submitted for Executive Director approval with a confirmed coverage plan. Neither Coordinator is obligated to cover the other's leave, and all arrangements are entered into voluntarily.
Primary Responsibilities
Service Coordination
Coordinate with MAiD providers, individuals, families, volunteers, designated funeral homes, home and community care services, and nursing agencies
Provide on‑site support and facilitation on the day of provision
Complete all required documentation, including booking paperwork, inquiry tracking, service data, and database records
Ensure the space is prepared, welcoming, and meets all needs
Manage and administer all aspects of accessing the space, providing guidance and support, including grief and bereavement resources
Offer follow‑up support and connect individuals and families with appropriate resources in the days and weeks following a provision, as needed
Contribute to operational procedures as requested, policy development rests with the Executive Director
Apply change, risk, and resource management principles as needed
Support and collaborate with other MAiDHouse sites as needed
Participate in research, evaluations,
and other quality assurance activities as requested
Relationship Management
Maintain responsive, skilled relationships with rostered MAiD providers and assessors; respond to inquiries within 24 hours on business days
Contribute to expanding the roster of providers and assessors affiliated with the Toronto site through proactive outreach
Respond promptly and professionally to inquiries from individuals, family members, healthcare facilities, and community organizations
Conduct tours of the space for eligible individuals, providers, assessors, and potential partners; donor tours are coordinated with the Executive Director
Engage in proactive community outreach during slower periods to increase awareness of MAiDHouse Toronto
Participate in local MAiD‑related networks, communities of practice, and qualified associations
Represent MAiDHouse professionally in all community interactions
Volunteer Coordination
Communicate expectations, logistics, and support needs to volunteers
May participate in volunteer training as directed by the Executive Director
Contribute to volunteer recruitment as requested by the Executive Director
Ensure volunteers feel valued and supported in their roles
Site Management
Maintain supplies and ensure the space is consistently stocked and ready
Track supply expenses and submit for reimbursement
Coordinate basic site maintenance and communicate repair needs to the Executive Director
Ensure confidentiality and security protocols are consistently followed
Documentation & Reporting
Maintain accurate inquiry tracking, program data, and database records
Complete all booking and provision documentation according to established systems, filed digitally on the shared drive
Complete and share rotation handoff notes with the other Community Care Coordinator at the start of each rotation change
Produce reporting as needed; keep all data logs and documentation current on an ongoing basis
Contribute to Board reporting when requested
Report any operational issues, site concerns, volunteer matters, or stakeholder concerns to the Executive Director promptly

Comfort with aging, illness, dying, and death - personal or professional experience navigating end of life
Understanding of grief and bereavement - awareness of how loss impacts people
Person‑centred care approach - prioritises dignity, choice, and individual needs
Knowledge of Ontario's healthcare system and community resources (preferred)
Knowledge of or familiarity with MAiD in Canada - including the history and evolution of MAiD legislation (Bill C‑14 and Bill C‑7), eligibility criteria, the assessment and provision process, and the roles of the various practitioners involved (asset)
Relationship management - proven ability to build trust and maintain professional relationships
Community outreach - experience connecting with diverse stakeholders
Service coordination or management - experience coordinating services or programs
Trauma‑informed approach - understanding of how trauma impacts people and ability to create safe, supportive interactions (asset)
Education
Social or community‑based services, health‑related field, management, or related
Equivalent combination of training, skills, and lived experience considered
Experience
Previous end‑of‑life work or volunteer experience (preferred)
Experience in program coordination, healthcare, social services, or community work
Demonstrated ability to work independently and collaboratively
Experience maintaining strict confidentiality
Required

Work Conditions
Work is performed at a physical MAiDHouse location in Toronto, Ontario (location is confidential; provided upon hire)
Hybrid work environment while provision facilitation and site‑related duties are in‑person; administrative, documentation, and coordination tasks may be performed remotely
Physical aspects include helping people with mobility challenges, lifting 30–40 lbs, rearranging furniture and equipment, and navigating multiple flights of stairs
Work involves regular engagement with emotionally charged situations and exposure to a wide range of emotions
Provisions are typically arranged 48–72 hours in advance; must be available to respond within that timeframe during rotation weeks
Weekend work is required within rotation weeks
Early mornings and late afternoons as needed by provision scheduling
All documentation is digital; no paper files are to be kept or stored at the Contractor's home
Compensation & Contract Terms
Annual Base Compensation: $38,000 – $40,000 (plus applicable HST), confirmed in writing prior to start
Training Rate: $35.00/hour (plus applicable HST) during the 4–6‑week onboarding period
Additional services outside core scope: $40.00/hour (plus applicable HST), by mutual written agreement
Hours: Approximately 20–30 hours per week during rotation weeks; availability for clarification and emergency backup during non‑rotation weeks
Schedule: Alternating Wednesday‑to‑Wednesday rotation with the second Community Care Coordinator
Contract Term: 6‑month independent contractor agreement, beginning fall 2026, with opportunity for a 6‑month extension confirmed in writing at month five
Structure: As an independent contractor, you are responsible for your own taxes and benefits
Leave: Unpaid; discussed collaboratively between the two Coordinators and approved in writing by the Executive Director
Compensation is tied to services rendered; no invoice is submitted for approved leave periods or for provisions not attended
