Become a full-time Case Worker with SLCS in our Supported Independent Living (SIL) Program. You will provide crucial support in a one-on-one setting to adults with disabilities and mental health needs.

This role involves working with clients to develop case plans and connect them to essential community resources. As part of an interdisciplinary team, you will assess client needs, educate them on life skills, and maintain qualified relationships with community organizations. Crisis intervention with clients who may become aggressive is a key aspect of this role, ensuring their safety and wellbeing.

Key Responsibilities:
• Develop individualized case plans for clients
• Connect clients to community resources and supports
• Educate clients on life skills for independence




• Assess client risks and care needs
• Maintain professional relationships within the community

Requirements:
• Post-secondary education from an accredited institution
• 1-3 years’ experience with developmental disabilities
• Experience managing potential aggression with de-escalation
• Valid G Class Driver’s License
• Standard First Aid/CPR Level C certification
