Primary Care Alberta is advancing a patient-centred approach that supports safe transitions from hospital to home. The Integrated Care Program (ICP) assists patients at high risk of readmission by providing enhanced discharge planning, ongoing follow-up, and coordinated post-discharge care. The program focuses on individuals with medical and psychosocial complexity, helping them remain connected to primary care and community supports after leaving the hospital.As an ICP Nurse, you play a key role in helping patients stay well at home and reducing avoidable emergency department visits and hospital readmissions. You will work closely with patients, families, physicians, and interdisciplinary teams to support successful transitions from acute care to community-based care.Based at Sturgeon Community Hospital, this provincial role offers the opportunity to collaborate with ICP Nurses across Alberta while contributing to the ongoing growth of the Integrated Care Program.The ICP Nurse completes comprehensive assessments, identifies patient goals and barriers, develops individualized transition plans, and supports discharge planning. Following hospital discharge, the ICP Nurse provides regular check-ins to monitor progress, reinforce care plans, identify emerging concerns, and help patients navigate the health system. A key focus of the role is facilitating attachment to a primary care provider and connecting patients to appropriate community programs, services, and social supports. By strengthening these connections and coordinating care across sectors, the ICP Nurse helps improve patient outcomes, enhance the patient experience, and support health system sustainability.
Description: As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach,



counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. You will provide safe, quality patient and family centered care while reflecting the shared vision and values of PCA.PLEASE NOTE: Provisional Permit holders will be paid $40.76 to $55.79 per hour. Required Qualifications: Completion of an accredited nursing education program.Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA).Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).
Additional Required Qualifications: Minimum 7 years of nursing experience across acute care, home care, primary care, and/or community settings. Demonstrated ability to build collaborative relationships and work effectively within interdisciplinary teams. Strong assessment, care coordination, communication, critical thinking, and time management skills. Knowledge of community resources and supports for vulnerable populations, including mental health, addictions, poverty, and domestic violence. Experience facilitating care transitions, complex care planning, and connecting patients to primary care and community services. Proficiency with electronic health records, including Connect Care, and Microsoft Office applications, Teams, and Zoom. Ability to work independently in a quick-paced environment, prioritize competing demands, and adapt to changing patient and system needs. Demonstrated commitment to patient- and family-centred care, continuous learning, and professional development. Must be able to travel within the community and perform the physical duties associated with the role-physically able to perform all duties in practice setting which include ability to lift and carry up to 15 pounds, climbing stairs, standing, squatting, and sitting for long periods at a computer screen.
Preferred Qualifications: Baccalaureate Degree in Nursing preferred; equivalent combinations of education and relevant community nursing experience will be considered. Experience in case management, transitional care, complex care planning, and HealthChange Methodology. Training in Nonviolent Crisis Intervention is an asset.
