Fraser Health is looking for a Relief Part Time Primary & Community Care Social Worker in Surrey, BC. This crucial role focuses on psychosocial assessments and collaborative care planning with the community.

As part of the Specialized Community Services Program, you will work within an interdisciplinary team to deliver therapeutic counseling and comprehensive services. The role involves managing cases, assessing client needs, and coordinating with community resources to enhance care. Key Responsibilities:
• Conduct comprehensive psychosocial assessments and interviews
• Provide therapeutic support and services to clients and families




• Collaborate with interdisciplinary teams in care planning

Requirements:
• Current full registration with the BC College of Social Workers
• Two years of experience in complex health care settings
