Become a vital part of Access Alliance Multicultural Health and Community Services as a full-time Nurse Practitioner in Danforth. Provide comprehensive care to diverse populations while working in a collaborative environment.

This permanent position within the Primary Care Team focuses on delivering primary health care to immigrants and marginalized communities. The successful candidate will employ their skills in assessment, diagnosis, and treatment planning to address clients' unique health needs. You will engage with clients of all ages and work in a quick-paced clinic environment while prioritizing high-quality care and community health equity.

Key Responsibilities:
• Conduct assessments, diagnoses, and referrals for clients
• Manage an independent caseload including treatment and follow-up




• Tailor care strategies for diverse populations, acknowledging health disparities
• Participate in community partnerships and health initiatives
• Maintain accurate client records for continuity of care

Requirements:
• Master's prepared RN (EC) required
• Active registration with the College of Nurses of Ontario
• Two years’ experience as a primary care Nurse Practitioner preferred
• Experience with newcomer populations preferred
• Strong cultural competency and interpersonal skills
