Full Time Permanent 37.5 Hour Per week
Superior Family Health Team is looking for a motivated Nurse Practitioner to join our collaborative, multi-disciplinary clinic. In this role, you’ll work alongside a team of allied health professionals and community partners to provide high-quality, patient-focused care. You will manage your own patient roster, delivering comprehensive primary care that includes acute and episodic treatment, health promotion, disease prevention, and chronic disease management. Collect and maintain accurate data and information in accordance with the Personal Health Information Act (PHIPA) and the requirements of any regulatory body standards. Effective listening, observation,
and facilitation skills to effectively assess the patient and provide appropriate treatment and consultation. Other duties may be assigned
Qualifications
Current registration with the College of Nurses in Ontario in the Extended Class in good standing
The ability to work in a rapidly changing workplace.
Demonstrated ability to organize, prioritize and multitask efficiently.
