This role prioritizes comprehensive, client-centered care for individuals and families. You will engage in diagnosing, treatment planning, and providing comprehensive follow-up and preventive care services. Solid collaboration within a Medical Home Model is essential for addressing varied health concerns.
Key Responsibilities:
• Assess and manage patients with acute and chronic conditions
• Order and interpret diagnostic tests as necessary
• Liaise with community resources and provide referrals
• Promote preventative health practices and immunizations
• Document client interactions clearly and accurately
Requirements:
• Master of Nursing – Nurse Practitioner Stream or equivalent
• Community-based primary care experience
• Ability to provide trauma-informed care
• Registration or eligibility with the Yukon Registered Nurses Association
• Current CPR and proven liability insurance
