AgeCare is seeking a Nurse Practitioner to collaborate on resident care within his/her level of competency. This role provides quality and protected resident care to support and align resident needs.
You will work with the MD, DOC and interdisciplinary team to assess, diagnose, and evaluate acute and chronic illness, prescribe treatments and follow-up care, and participate in care conferences and educational programming for team members, residents, and families.
