As a Registered Practical Nurse (RPN), you'll conduct scheduled visits to assess and meet patient needs, working under the guidance of Registered Nurses for complex care scenarios. Your expertise will shine as you foster long-term relationships and deliver ethical nursing care while maintaining comprehensive patient records.

Key Responsibilities:
• Conduct community visits to address patient care needs
• Provide safe and ethical nursing support in collaboration
• Use assessment tools to evaluate patient requirements
• Monitor and adjust ongoing care plans as necessary
• Build rapport with patients to foster trust and comfort

Requirements:
• Registration with the CNO as an RPN
• Valid driver’s license and access to a vehicle
• Experience in community care is preferred
• Flexibility and reliability in work hours
• Passion for serving vulnerable populations
