Enhance community health at South Riverdale Community Health Centre as a full-time Program Manager for Chronic Disease. Oversee initiatives that tackle diabetes and other chronic conditions while managing a dedicated team.

In this role, you will ensure effective program delivery and support for clients with chronic health needs. Your responsibilities include supervising staff and collaborating with community partners to develop impactful health services. Familiarity with health promotion strategies will aid in achieving program goals.

Key Responsibilities:
• Supervise interdisciplinary teams and manage program staff
• Evaluate and ensure adherence to best practice guidelines
• Develop strong inter-professional programming and partnerships




• Monitor service delivery and client care effectively
• Prepare comprehensive funding proposals and reports

Requirements:
• Graduate degree or relevant health care experience
• 2-5 years of program management experience
• Registered health care skilled preferred
• Demonstrated experience with vulnerable populations
• Excellent verbal and written communication skills

Lead impactful chronic disease programs that prioritize community health and well-being at SRCHC.
