Join Stonehenge as an Intensive Case Manager, focusing on crisis stabilization for mental health and addiction clients. This full time position requires managing cases and providing essential support in a 24-7 environment.

As a key member of an interdisciplinary team, you will coordinate care and provide intake services for clients experiencing crises. You'll oversee client transport, case management, and support for stabilization efforts. Evening and weekend shifts are necessary, along with on-call responsibilities.

Key Responsibilities:
• Process referrals and assess program fit
• Conduct intake and enrollment activities
• Develop and implement individualized care plans
• Support clients in accessing health services
• Monitor the Crisis Stabilization building

Requirements:
• College Diploma or University Degree in relevant field
• Minimum one year in addiction or mental health
• Experience in concurrent disorders an asset
• Valid Ontario driver’s license and reliable vehicle
• Standard First Aid/CPR Level C certification
