Key Responsibilities:
• Collaborate on individualized wellness plans for clients
• Use standardized tools to assess client health
• Offer person-centered access to comprehensive services




• Develop robust community resource collaborations
• Maintain accurate records and ensure compliance with agency policies

Requirements:
• Degree in Social Work, Psychology, or similar field
• At least 2 years in Mental Health management
• Experience with responsible mental illnesses and dual diagnoses
• Excellent interpersonal and crisis intervention skills
• Canadian driver’s license required for travel
