Under the Program Manager’s direction, the Community Health Facilitator plays a crucial role in client service delivery from intake to ongoing support. Responsibilities include assessing client needs, coordinating resources, and providing vital education within the community. Key Responsibilities:
• Facilitate recent client intake and eligibility assessments
• Coordinate with outreach workers and program activities




• Provide education about client challenges and local resources
• Support tenant engagement including lease negotiations
• Build lasting relationships with local landlords

Requirements:
• Degree/diploma in Health or Social Services
• Minimum five years of client service experience
• Excellent organizational and facilitation skills
• Valid Class V driver's license required
• Understanding of Indigenous cultures and challenges
