In this impactful position with the Primary Care Network, you will support clients by assessing needs and coordinating treatment plans. Your expertise in mental health will facilitate access to resources and services while promoting hope and recovery. Collaborate closely with interdisciplinary teams to ensure comprehensive care.
Key Responsibilities:
• Conduct assessments and create short-term treatment plans
• Facilitate connections to community resources and services
• Maintain detailed client documentation and statistics
• Advocate for clients in accessing support related to living essentials
• Deliver informative sessions on community resources
Requirements:
• Master’s degree in Social Work or related areas
• At least two years’ field experience in psychiatry
• Valid B.C. Driver’s license necessary
• Strong practical knowledge of CBT and mental health issues
• Ability to utilize standardized assessment techniques
