Key Responsibilities:
• Coordinate treatment plans with patients and community partners
• Conduct psychosocial assessments for improved discharge processes

Requirements:
• Bachelor’s of Social Work from a recognized institution
• Registration with the Ontario College of Social Workers
• Valid Ontario Driver’s License
• Expertise in crisis intervention and rehabilitation practices
• Robust analytical and communication abilities
