Key Responsibilities:
• Lead comprehensive primary care services for residents
• Complete assessments and medication reconciliations
• Collaborate with physicians for integrated care decisions




• Provide clinical coaching and educational support
• Facilitate palliative care and end-of-life discussions

Requirements:
• Valid CNO registration as Nurse Practitioner
• Experience in gerontology or long-term care preferred
• Strong communication and consultative skills
• Fulfill legal requirements and background checks
• Maintain vaccination and health documentation
