Become a vital part of Fatima House as a Full time Mental Health and Addictions Case Manager. This role emphasizes harm reduction and mental health support while working a 37.5-hour week from Thursday to Monday.

As a Case Manager at Fatima House Women’s Hostel, you'll promote client recovery through mental health services and advocacy. Your expertise will be crucial in providing outreach and harm reduction strategies, while ensuring a supportive and inclusive environment. This role requires engaging directly with vulnerable populations and developing tailored service plans.

Key Responsibilities:
• Implement harm reduction and client-centered recovery services
• Conduct community outreach and advocate for client resources
• Document client progress and adjust service plans accordingly




• Collaborate and support external agency staff while on-site
• Provide crisis intervention and de-escalate stressful situations

Requirements:
• Minimum 3 years experience in mental health or addictions
• Educational background in social services with a focus on harm reduction
• Knowledge of community resources and supportive housing
• Strong crisis intervention and conflict resolution skills
• Ability to work shifts and maintain accurate records
