DESCRIPTION OF TOWN/CITY

The position will work out of the Crosstown Family Health Team site, which is situated in the Mount Dennis neighbourhood in Toronto, a multicultural city. Mount Dennis has been designated as a high-needs community and is served by the TTC and GO Transit.

DESCRIPTION OF PRACTICE

Crosstown Family Health Team is an interdisciplinary primary care organization serving individuals and families in the Weston/Mount Dennis community. Our team works collaboratively to provide accessible, comprehensive primary care to a diverse patient population.

POSITION SUMMARY

Under the direction of the Executive Director, the Facilities and Maintenance Worker is responsible for supporting the general maintenance, safety, functionality, and upkeep of the Crosstown Family Health Team clinic and associated spaces.

The successful candidate will work independently while also collaborating with clinic staff, contractors, landlords, and external service providers as needed.

Responsibilities will include, but are not limited to:

- Attending to maintenance and repair issues for clinic grounds and equipment in a timely manner.
- Performing painting, drywall, plumbing, flooring, structural, HVAC, door/lock/window, electrical repair as required.
- Assemble, install, repair, and relocate furniture, shelving, fixtures, and equipment.
- Conduct regular clinic audits to identify maintenance, repair, or safety concerns in a proactive manner.
- Maintain storage, utility, staff, and common areas in a safe and organized manner.
- Attend to seasonal/outdoor maintenance including but not limited to clearing walkways of debris/snow,



ensuring roof/gutters/greenhouse/fencing/sump pumps/drains remain in good repair
- Support small facility improvement and renovation projects.
- Coordinate with and provide access to contractors and service providers as required.
- Identify maintenance issues that require a licensed trade or specialized contractor and report these to the Executive Director.
- Ensure deliveries promptly received and moved to place of use.
- Ensure the clinic remains safe and functional for patients and staff.
- Perform other related duties as required.

Successful candidates will be reliable, practical, and comfortable working independently in an active healthcare environment. The position requires good judgment, attention to safety, and the ability to prioritize and address a variety of day-to-day facility needs.

QUALIFICATIONS

- Previous experience in facilities maintenance, building maintenance, general repairs, handyman services, or a related role.
- Experience completing basic repairs and maintenance using common hand and power tools.
- General knowledge of building maintenance and repair.
- Ability to troubleshoot minor maintenance issues and determine when a licensed tradesperson or specialized contractor is required.
- Strong organizational and time-management skills.
- Ability to work independently and manage competing priorities.
- Good communication and interpersonal skills.
- Reliable and responsive to the operational needs of the clinic.
- Ability to perform the physical requirements of the position, including moving supplies, furniture, and equipment and completing general maintenance tasks.

Reports to: Executive Director or his/her designate

Hours of Work: Approximately 0.6 FTE, with schedule to be determined

Status: Contract, with possibility of extension

Work Location: In person
