ABOUT THE OPPORTUNITY

NL Health Services – Central Zone invites applications from Family Physicians to join the Family Care Team (FCT) Program in a full-time salaried position in the Lewisporte Region.

Central Zone is the second largest health zone in NL Health Services, serving approximately 90,000 people across 176 communities. The region extends from Charlottetown in the east, Fogo Island in the north, Harbour Breton in the south, to Baie Verte in the west — encompassing more than half the total land mass of the island of Newfoundland.

The Family Care Team mandate is to provide patients without a primary care provider with access to primary health care services using an interprofessional collaborative approach,



where patients see the right provider at the right time and place. The team may include physicians, Nurse Practitioners, Registered Nurses including a diabetic nurse educator, clerical support, LPNs, pharmacists, social workers, dietitians, physiotherapists, and occupational therapists. All health care providers, including physicians, are expected to practice to their full scope of practice to meet patient needs. The FCT aims to provide continuity of care to improve health outcomes and patients' quality of life.

The successful candidate will manage a roster of patients in addition to supporting work at the Category B facility within the community. Additional support of Long-Term Care and Personal Care Home services is also part of the responsibilities. Hours of work may require flexibility to offer clinics during daytime or coverage of Category B Emergency. After-hours and/or weekend coverage may be required. Care will be provided using the EMR platform.

Work Environment

QUALIFICATIONS

- Medical Degree (MD) from a recognized institution
- Eligible for licensure as a Family Medicine Physician with the College of Physicians and Surgeons of Newfoundland and Labrador (www.cpsnl.ca)
- Must satisfy credentialing requirements of NL Health Services including a satisfactory criminal record check and vulnerable sector check

DUTIES & RESPONSIBILITIES

- Manage a roster of patients and provide comprehensive primary care services within the Family Care Team model




- Support work at the Category B facility within the community including Emergency coverage as required
- Provide additional support to Long-Term Care and Personal Care Home services as part of the team
- Deliver clinics during daytime hours and provide after-hours and/or weekend coverage as required
- Practice to full scope of practice within the interprofessional team-oriented team model
- Utilize the EMR platform for all patient care documentation
- Work collaboratively with the interdisciplinary Family Care Team
- Support person- and family-centered care; promote patient and worker safety by integrating safety into work practices, identifying risks, and implementing improvements

REMUNERATION

As per Memorandum of Agreement

Step assignment determined by the Department of Health and Community Services in accordance with Provincial Guidelines, with yearly progression thereafter.
