Community Disability Services Practitioner

Position Summary

The Community Disability Service Practitioner (CDSP) plays a vital front‑line role in enhancing inclusive, person‑centered support and high‑quality program delivery at Chrysalis. Reporting to the Team Lead, this position provides direct support, facilitates meaningful programming, mentors staff, and contributes to collaborative service planning. The ideal candidate is compassionate, skilled in relationship‑building, and committed to fostering independence, inclusion, and personal growth for Participants.

What You’ll Do!

- Provide daily support, personal care, life‑skills development, and confidence‑building for Participants at Chrysalis.
- Support the setup, delivery, and adaptation of scheduled programs to ensure meaningful participation.
- Foster respectful interactions, encourage social connection, and model positive communication.
- Maintain accurate records, prepare for meetings, and communicate with guardians and team members on a day to day basis.
- Mentor CDSWs, support onboarding, and contribute to consistent, high‑quality service delivery.
- Implement behaviour strategies, apply trauma‑informed approaches, and support emotional wellness.
- Uphold confidentiality, professionalism, and inclusive workplace values.

What We Need From You!

- Proven experience in supporting individuals with disabilities and complex needs.
- Commitment to person-centered support and inclusion.
- Ability to effectively communicate with individuals with diverse needs, families, and colleagues, both verbally and in writing.
- A compassionate approach to working with individuals, understanding their challenges,



and providing supportive guidance.
- Willingness to engage in a variety of programs (swimming, physical activity, arts, community outings).
- Strong ability to manage caseloads, document progress, and track outcomes to ensure clients receive the appropriate services and support.
- Ability to collaborate effectively with multidisciplinary teams, including CDSW’s, Team Leads, and Coordinators.

Skills and Qualifications

- Education: Post‑secondary education in Disability Studies, Social Work, Community Rehabilitation, Human Services, or a related field (asset).
- Experience: Experience supporting individuals with disabilities in community or program settings and experience mentoring or supporting team members (asset).
- Technical Skills: Personal care and mobility assistance, behavior support and de-escalation, health and safety, promoting independence, and documentation and reporting using Google Suite and MyCompass.
- Knowledge: Person centered support principles, trauma informed practice, behaviour support strategies and understanding boundaries confidentiality and ethical practice.
