We're seeking a Home Support Supervisor to oversee community-based nursing care in Peel, North York, Scarborough, York, and Toronto. As a Registered Practical Nurse, you'll lead and develop your PSW team. Your primary focus will be on patient assessments and setting care goals while ensuring your team is well-trained and supported. You will manage staff performance and maintain awareness of key indicators that affect service quality and patient satisfaction. Key Responsibilities:

- Conduct home visits to assess patient needs
- Formulate and implement care plans with the team
- Manage staff through performance evaluations
- Record and analyze feedback for service improvement
- Address patient concerns efficiently Requirements:
- Must be registered with CNO in positive standing
- At least three years of direct supervisory experience
- Valid driver's license and access to transportation
- Strong solution-oriented interpersonal skills
- Proficient in English communication, additional languages are a plus Lead by example and drive healthcare excellence as a Home Support Supervisor.
