Licensed Practical Nurse (LPN) - CHRONIC DISEASE MANAGEMENT (CDM) NURSE The CDM Nurse will provide comprehensive nursing services to the patient population of family practice clinics within the Primary Care Network. This role will assess, manage, and support patients and work collaboratively with the physicians to develop, implement, and evaluate care and treatment plans for those patients with chronic disease.

Our ideal candidate has:

· A minimum of 3-5 years of Nursing experience

· Experience in some or all of the following areas is desirable:

·Primary Care/Community Nursing ·Chronic Disease Management

· Additional education in chronic disease management is desirable

· Advanced Footcare Certification is an asset

· Skills in counselling, group facilitation, community development, teaching, and presentation

· Excellent interpersonal and public relation skills; ability to foster relationships with physicians, patients, and families

· Strong problem solving and evidence based decision-making skills





· Teamwork and relationship building skills

· Planning, organizing, multi-tasking, time management and prioritizing skills

· Respect and understanding of privacy legislation, including the Health Information Act

· Knowledge of Microsoft, NetCare, Electronic Medical Records

· Completion of an accredited Licensed Practical Nursing program with current CLPNA registration

· Current health care provider CPR certification

Pay: $32.19-$40.12 per hour
