: This position will be part of the Intermediate Care Services within the Primary Health Care Networks. This service addresses a provincial health system service gap, where some clients with multiple complex health issues have difficulty in accessing an office based primary care provider.
The goals for this service are: to provide the most at risk clients with home based access to higher level health care providers, including MDs and NPs; to mitigate the risk for worsening health sates and acute care episodes leading to Emergency department visits and hospitalizations for at risk clients; to reduce system pressures on acute care services, particularly Emergency departments and hospitals; to provide cost effective and high quality health care using patient centered and community based approach. Practitioners in this service are assigned to one of the Primary Health Care Networks and provide support to the interdisciplinary Primary Health Care team.

Experience
- Minimum of two (2)
years recent related nursing experience is required
Education
- Current registration with CRNS and current BCLS certification.
Licenses
- Valid Class 5 driver's license
- Registered Nurse (RN)
- Ability to demonstrate appropriate level of qualified conduct and judgement.
- Has demonstrated strong interpersonal and communication skills.
- Demonstrates application of the nursing process
- Ability to initiate and exercise critical thinking and decision-making
- Ability to exercise independent nursing judgement
- Current clinical knowledge and skill are required in a broad range of areas.
- Ability to promote effective relationships within the SHA and the community.
Other Information
- Community nursing experience is an asset
- Registered/eligible with the College of Registered Nurses of Saskatchewan (CRNS)
- Must possess a valid driver's license and have a vehicle available for work.
- Willing to Travel to rural areas as assigned. Travel allowance provided.

- This is a community-based position providing services in clinic within the Primary Health Care Networks (Regina) and can be assigned to one of the Primary Health Care Networks. Home base: 2180 Victoria Ave. E.
