Description The Scarborough Academic Family Health Team (SAFHT) is seeking a qualified and experienced individual to join our team as a Registered Nurse (RN). SAFHT has 4 new RN roles that will report to the Executive Director and/or manager and will work our interdisciplinary team. The successful candidates will primarily work within one of SAFHT’s affiliated physician offices and collaborate closely with the supervising physician and interdisciplinary team to provide comprehensive, patient-centred primary care services.

The RN may be required to travel between SAFHT sites as operationally required.

Working to full scope of practice, the RN provides comprehensive nursing care to patients across the lifespan within a primary care setting. Embedded within an affiliated physician practice, the RN collaborates closely with physicians and the interdisciplinary team to provide timely access to high-quality, evidence-informed care.

The RN practices independently within the College of Nurses of Ontario (CNO) standards and utilizes organizational policies, medical directives, and clinical practice guidelines to deliver protected, effective, and patient-centred care. The role supports prevention, health promotion, chronic disease management, episodic care, care coordination, and quality improvement initiatives.

About Us:

The SAFHT is governed by a Mixed Board (i.e. Physicians and Community Members) and is an integrated group of health care providers that work collaboratively as part of a family health team. Our interprofessional care team currently includes 29 affiliated physicians, nurse practitioners, registered nurses, registered dietitians, a case manager, social workers, chiropodists, and several administrative staff.

Originally formed in 2007, the SAFHT serves patients by providing them with timely access to a comprehensive range of services and programs aimed at managing and preventing disease and improving health status. Including our SAFHT affiliated physician group locations, we deliver care across 8 locations in the Scarborough and Eastern Toronto geography and engage in teaching learners in medicine and other health disciplines. Role Responsibilities:

In collaboration with physicians and the interdisciplinary team, the Registered Nurse:

- Performs comprehensive nursing assessments, including health histories, physical assessments, medication reconciliation, and risk assessments.
- Assist with onboarding new patients
- Assesses, monitors, and manages patients with stable chronic diseases including diabetes, hypertension, COPD, asthma, heart failure, osteoporosis, chronic kidney disease, and other common primary care conditions within RN scope of practice.
- Provides assessment and nursing management for acute episodic illnesses and determines when consultation with the physician or nurse practitioner is required.
- Performs nursing procedures including immunizations, injections, wound care, ear irrigation, ECGs, point-of-care testing, specimen collection, suture and staple removal, and other procedures within scope and organizational policies.
- Supports preventative care initiatives including cancer screening, cardiovascular risk reduction, smoking cessation, sexual health, healthy lifestyle counselling, and adult and childhood immunization programs.




- Delivers evidence-based patient and family education to promote self-management of chronic diseases and healthy behaviours.
- Participates in medication monitoring and follow-up for patients with chronic conditions.
- Coordinates care with specialists, hospitals, home and community care providers, pharmacies, and community agencies to facilitate continuity of care.
- Participates in preventive care outreach initiatives, recall programs, and follow-up of patients requiring ongoing monitoring.
- Utilizes evidence-based clinical practice guidelines to support high-quality patient care.
- Documents assessments, interventions, care plans, and patient encounters accurately and in a timely manner within the TELUS Practice Solutions Suite (PSS) electronic medical record.
- Maintains accurate statistical reporting and contributes to Ministry reporting requirements where applicable.
- Participates in interdisciplinary case conferences, quality improvement initiatives, program development, and organizational planning.
- Supports learners and contributes to SAFHT's academic and teaching environment where appropriate.
- Maintains patient confidentiality and complies with all organizational policies, privacy legislation, and infection prevention and control standards.
- Other tasks as operationally required.

Accountability The Registered Nurse reports to the Executive Director and/or the manager of SAFHT. The Registered Nurse practices in accordance with:

- College of Nurses of Ontario Professional Standards
- Nursing Act, 1991
- Regulated Health Professions Act, 1991
- Relevant Ministry of Health policies and clinical guidance
- SAFHT policies, procedures, and approved medical directives

Qualifications

- Current registration in good standing with the College of Nurses of Ontario as a Registered Nurse.
- Bachelor of Science in Nursing (BScN) preferred.
- Minimum two years of recent nursing experience, preferably in a Family Health Team, primary care, community health, ambulatory care, or outpatient setting.
- Demonstrated knowledge of comprehensive primary health care across the lifespan.
- Experience assessing and managing patients with chronic diseases using evidence-based clinical practice guidelines.
- Demonstrated excellence in health promotion, disease prevention, patient education, and self-management support.
- Strong assessment, critical thinking, clinical decision-making, and care coordination skills.
- Demonstrated ability to work both independently and collaboratively within an interdisciplinary team.
- Excellent communication, interpersonal, organizational, and time management skills.
- Experience using electronic medical records; proficiency with TELUS Practice Solutions Suite (PSS) is considered an asset.
- Proficiency with Microsoft Office applications.
- Current Basic Life Support (BLS) certification.
- Demonstrated commitment to continuous quality improvement and evidence-informed practice.

Assets Preference may be given to candidates with one or more of the following:

- Certified Diabetes Educator (CDE)



or Certified Diabetes Care and Education Specialist (CDCES) designation.
- Canadian Nurses Association (CNA) Certification in Primary Care Nursing or Community Health Nursing.
- Smoking cessation counselling certification or experience.
- Experience providing cervical cancer screening (Pap testing), where applicable.
- Experience in wound care
- Experience with spirometry or pulmonary function testing.
- Experience in cardiovascular risk assessment and management.
- Experience in immunization programs, vaccine management, and cold chain maintenance.
- Experience participating in Quality Improvement initiatives, accreditation activities, and program evaluation.
- Ability to communicate in languages reflective of Scarborough's diverse communities.

Other Responsibilities

- Accepts referrals and delegated clinical activities from affiliated physicians in accordance with CNO standards and organizational medical directives.
- Works to full RN scope of practice, utilizing approved medical directives where appropriate to improve timely access to care.
- Provides nursing follow-up related to laboratory investigations, diagnostic imaging, preventive screening, chronic disease monitoring, and medication management.
- Assists physicians with complex patient care planning and coordination.
- Supports same-day and urgent access appointments within nursing scope of practice.
- Collaborates with interdisciplinary health professionals to optimize patient outcomes.
- Assists in the planning, implementation, and evaluation of health promotion and chronic disease management programs.
- Maintains collaborative relationships with hospitals, community organizations, Ontario Health Teams, and other health system partners.
- Participates in quality improvement, accreditation, organizational committees, and program evaluation activities.
- Supports clinical education for students and contributes to the academic mandate of SAFHT where appropriate.
- Maintains qualified competence through continuing education and ongoing professional development.
- Provides timely reports and statistical information as required.
- Performs other duties as operationally required.
