Job Summary

We are seeking a compassionate and dedicated Rehabilitation Support Worker (RSW) to provide exceptional care and assistance to individuals in need of support with daily activities. In this vital role, you will work closely with clients to promote their independence, safety, and well-being within a private home setting. You will help them integrate into the community.

Responsibilities

- Assist clients with daily living activities
- Provide companionship and emotional support to reduce feelings of loneliness, anxiety or isolation
- Community reintegration (ie: Shopping; library; leisure walks...)
- Accompany the client to their local gym; assist with exercise routine as needed
- Monitor and report any changes in health status or behavior to healthcare professionals or family members

Experience

- Prior experience is highly preferred




- Demonstrated experience caring for individuals with disabilities, including those with cognitive impairments
- Knowledge of medication administration procedures and safety standard
- Familiarity with providing compassionate care for individuals with various disabilities
