This Transplant Licensed Social

Worker will work directly with transplant patients at St Joseph's Hospital in Tampa, FL.

The Transplant Licensed Social Worker Responsibilities Include

- Act as member of collaborative practice model that includes patients, nurses, clinical resource coordinators, physicians, and other practitioners, caregivers, and the community
- Psychosocial Assessments:
- Conduct thorough psychosocial evaluations to assess a patient's readiness for transplant surgery, recovery and rehabilitation
- Evaluating social support systems, family stability, adherence to medical advice, lifestyle behaviors and understanding of transplant risks and advantages.




- Identify potential barriers to transplant eligibility and help develop individual care plans.

- Post-Transplant Support:

- Provide ongoing counseling and resources to promote adherence to medication regimens, lifestyle adjustment and emotional well-being.
- Provide support to reduce anxiety, depression and potential substance misuse, improving long-term outcomes.

- Quality Improvement Duties:

- Develop clinical protocols
- Implement quality improvement initiatives

Certifications And Licensures

- Required FL LCSW (Social Work)

Education

- Required Masters in Social Work

Experience

- Required 2 years' experience as Licensed Social Worker specific to Transplant Services
