The hiring rate is dependent upon several factors, including but not limited to education, training, work experience, terms of any applicable collective bargaining agreement, seniority, etc.Job SummaryThe Clinical Social Worker coordinates the development and implementation of a patient care/treatment plan through use of diagnosis and clinical assessment of psychiatric, chemical dependency and psychosocial needs as they relate to medical and behavioral care needs. Provides disposition and discharge planning, risk assessment, case management, and coordination of services to provide continuity of care.ResponsibilitiesComplete appropriate intake documentation, provide ongoing assessment, develop treatment plan in collaboration with the multi‑disciplinary treatment team. Manage multiple complicated cases, maintain current knowledge of community resources, monitor patient needs and progress, develop aftercare plans. Maintain documentation on patient throughout hospitalization.Provide brief therapeutic interventions with patients and/or families, as well as community staff, in coordination with the physician and treatment team. Work as an integral member of the treatment team in the provision of treatment and aftercare planning.Ensure compliance with privacy and confidentiality laws. Interface regularly with legal entities to address treatment needs.Assist with monitoring and evaluation of services provided for department quality improvement plan.Maintain current understanding of issues related to culture, ethnicity, gender identity, age, sexual orientation, religion and utilize appropriate communication skills.Provide information,



education and direction in response to community requests.Regularly access pharmaceutical and community resources to provide patients with needed psychiatric medications.Diagnose, assess and address both mental health and chemical dependency conditions. Perform risk assessments for suicidality, homicidality and grave disability.Screen and identify patients who need care management per high‑risk criteria.Assess, develop, implement and monitor a comprehensive discharge plan of care through an interdisciplinary team process in conjunction with the patient and family. Collaborate with the multi‑disciplinary team to identify problems or needs that require special planning, intervention, teaching or follow‑up.Perform other duties as assigned.What you bringMaster’s Degree in Social Work from an accredited graduate school of social work.2 years of clinical experience with the designated population.Preferred experience in hospital, emergency room, chemical dependency, or community agency settings.Licensure as a Clinical Social Worker: LICSW (WA), SWAL (WA), SWAICL (WA), or SWAAL (WA).SkillsFamiliarity with medical terminology and conditions as they relate to discharge needs.Excellent verbal and written communication skills.Superior ability to work collaboratively within a team.Excellent organizational skills.Ability to document on electronic record.Knowledge of community resources, Medicare, Medicaid, and insurance requirements as they relate to continuing care (preferred).BenefitsFor positions less than 0.5 FTE: 403(b) retirement plan for caregiver contributions; wellness perks;discount program; expanded EmployeeAssistance Program and mental health program.EEOPeaceHealth is an Equal Opportunity Employer.
