The Coordinator responds to inquiries from key stakeholders directly and/or indirectly and assists the Director by researching and providing requested data or background information, as requested. The Coordinator problem solves barriers to transitions in care by working closely with the Access to Care & Transitions Specialists. The Coordinator completes daily, weekly and monthly summaries provided to relevant "leaders in the hospital" and is fully informed of issues and challenges that require action. Utilizing sound clinical judgment and strong assessment skills, the Patient Flow Coordinator screens and evaluates potential candidates for admission, collaborating with patients, families, care teams, and community organizations to coordinate person-centered transitions. The role requires adaptability and responsiveness to evolving system pressures and patient needs, often supporting flow initiatives as they evolve.Skills And Qualifications RequiredGraduate of a recognized school of PhysiotherapyActive, unrestricted license to practice with the College of Physiotherapists of OntarioExperience with electronic patient record systemsTwo (2) years of experience in providing clinical practice in a hospital setting with the relevant population. Intake and discharge planning functions, preferredDemonstrated understanding and alignment to current healthcare funding models, expectations and deliverables of Providence Care Hospital,



including achievement of efficient patient flow and targeted lengths of stayValid Ontario Driver's license, clear Driver's Abstract and access to a reliable vehicle as necessaryPosition ResponsibilitiesLeads Bed Utilization Meetings and other key meetings, as assigned, to facilitate communication and coordination related to patient flowReviews Daily Dashboard and clarifies data quality issuesTriages inpatient referrals for appropriate and timely review by respective inpatient Access to Care and Transition specialistProblem solves barriers to transitions in care at admission and transitions in careProvides daily and weekly summaries to Providence Care leadershipReview trends, variances and reasons for variances for admissions, transfers and dischargesEnsures appropriate and accurate data entry in support of safe transitionsProvides real-time resolution of issues brought forward regarding barriers to admissions, discharge refusals, repatriations and community support servicesFacilitates community referrals to support efficient and effective discharge planning and/or transition as necessary (this includes but is not limited to community agencies such as Home and Community Care, Addiction and Mental Health Services)Takes a primary role in managing patient/client/family/provider expectations concerning admissions and discharges and communicating discharge logistics to achieve timely transitions, positive outcomes,
