Position:GAIN Community Team Occupational TherapistPosition Type:Full-time Permanent (On-site)Hours of work:35 hours per weekCompensation:$40 HourlyDate posted:May 20, 2025Position Summary:The Occupational Therapist in the Geriatric Assessment Intervention Network (GAIN) Community Team is responsible for providing assessment utilizing various standardized tools with clinical judgment and developing interventions for older adults and seniors affected by emotional, psychological and physical impacts of aging.ResponsibilitiesParticipate as geriatric assessor in inter‑qualified Comprehensive Geriatric Assessment (CGA)Complete clinical triage for new referralsData gathering and recording within all domains of Comprehensive Geriatric Assessment (CGA)Provide clinical care and consultation services to improve and/or maintain activities of daily living for frail older adults and seniorsAnalyze frail older adults and seniors’ capabilities and expectations related to life activities through observation, interviews and formal assessmentsCollaborate with the GAIN inter‑qualified team to establish care plans for patientsQualificationsUniversity degree in Occupational TherapyRegistered and in good standing with the College of Occupational Therapists of OntarioRegistration with CAOT (Canadian Association of Occupational Therapists) or OSOT (Ontario Society of Occupational Therapists) is an assetCurrent CPR and First Aid certificationTwo to three (2‑3)



years’ working experience in the fieldDemonstrated extensive knowledge and skill working with individuals with various types of dementia, delirium, mental health, addictions, responsive behaviours, other neurological conditions, and those who experience an acute behavioural changeExperience and knowledge of working therapeutically with geriatric populations is an assetDemonstrated sound knowledge of cognitive and mobility assessment toolsCertification in Gentle Persuasive Approaches, PIECES or Montessori preferredSkill at crisis intervention and assessment preferredProficiency in other languages desired (Armenian, Greek, Mandarin, Tamil, Hindi, Urdu, etc.)Vulnerable sector check requiredA valid driver’s license and a car required for home visitsBenefitsCompetitive compensation and benefitsRewarding career that supports meaningful work in our communitiesTraining and professional development opportunitiesHealthy and safe working environmentEqual Employment OpportunitySenior Persons Living Connected is an inclusive and diverse work environment.
