This role focuses on delivering primary care to older adults and LTC residents—many of whom present with complex chronic conditions and mental health needs. You will work collaboratively within an inter-professional healthcare team, providing assessment, treatment, and ongoing management for a broad range of medical conditions. The LTC home operates within a community clinic setting, integrating primary care with geriatric and mental health components. This position also supports a recently implemented Mobile Health Clinic (Healthcare on Wheels), extending care directly into the community via a mobile van and other health hubs.Key ResponsibilitiesProvide assessment, diagnosis, treatment,



and follow-up care for acute and chronic illnessesDeliver primary care services to LTC residents and geriatric patientsManage complex chronic conditions and age-related health concernsProvide care for patients presenting with mental health concerns commonly associated with geriatric populationsConduct comprehensive health assessments and develop individualized, patient-centered care plansProvide medication management and monitoringSupport patients and families through education, care planning, and treatment managementParticipate in peer/chart audits and quality improvement initiativesWork collaboratively with physicians, nurses, and allied health professionals within an inter-professional care teamProvide telephone consultation and follow-up care as requiredRequirementsEducationMaster’s prepared Nurse Practitioner with Primary Health Care CertificateCertificationsRegistration in good standing with the College of Nurses of Ontario – Extended ClassMember of NPAO with professional liability coverageExperience3+ years of Nurse Practitioner practice experienceExperience in primary care strongly preferredExperience in long-term care, geriatrics, or senior care settings preferredExperience working with mental health and addiction populations considered an assetTechnical SkillsDemonstrated ability to work independently with strong clinical decision-making skillsExcellent communication and interpersonal skillsStrong organizational and care coordination abilitiesValid driver’s license and access to a vehicle requiredCandidate FitThis role is ideal for a self-directed NP with a genuine passion for geriatric and community-based care.
