Care Designers are self‑led leaders who take ownership of outcomes, thrive in a collaborative environment, and are inspired by building meaningful relationships with seniors, families, caregivers, and community partners.Key ResponsibilitiesConsult Conversion & Community EngagementConduct Caring Consults, assessing clients’ physical, emotional, and environmental needsDevelop and recommend personalized care solutions aligned with client goalsAchieve a 90% consult conversion rateAct as a trusted advisor to clients and their familiesBuild and maintain relationships with community leaders and skilled referral partnersPromote high‑quality, innovative customer service and Nurse Next Door’s brand promiseClient Care Delivery & ExperienceAttend all First Visits to ensure care plans and service agreements are executed properlyConduct supervisory visits to ensure caregiver fit and quality of careDevelop, implement, and update individualized care plansEducate clients on self‑care and evaluate responses to careMonitor risks,
identify trends, and adapt care as neededCoordinate care and evaluate outcomes based on client goalsDeliver a premium client experience with a target Net Promoter Score (NPS) of 9+People LeadershipLead, coach, and support the caregiving team to achieve resultsSet clear goals, provide ongoing feedback, and coach in the momentRecruit, train, and manage caregiver performance and availabilityFoster a continuous learning environment, including Discovery Day and monthly in‑service programsConduct First and supervisory visits to evaluate caregiver performance and engagementDrive a positive employee experience with a target employee NPS of 9+Scheduling CollaborationPartner with the scheduling team to communicate changes in client needs and caregiver assignmentsEnsure consistent client coverage, including urgent care needs and caregiver cancellationsSupport scheduling as needed to meet operational requirementsMaintain AMS (Attendance Monitoring System – AlayaCare) below 5%Ensure data accuracy and completeness in scheduling systemsAdministration & ComplianceMaintain accurate and complete documentation, including care plans, client files, and employee recordsConduct quarterly audits to ensure 100% accuracy and complianceEnsure compliance with provincial and federal legislation related to health, employment,
