In this position, you’ll use your clinical expertise to connect with patients living with diabetes, conducting MedsChecks, dispensing and optimizing their medication therapy, and empowering them through education and support.What You’ll DoReporting to the Pharmacy Manager, the Pharmacist will support the pharmacy by providing diabetes education, training, comprehensive medication assessments, and medication therapy management services for patientsBuild rapport and foster trusted, ongoing patient relationships over the phone and in-personSupport all responsibilities related to pharmacy operations including but not limited to medication/supplies dispensing and administrationCollaborate with prescribers and other healthcare providers to ensure coordinated careDocument consultations and follow-ups in compliance with standards of practiceStay current with diabetes guidelines and best practices to deliver evidence-based careWhat We’re Looking ForInterest in helping people with diabetes improve their health and wellbeingOCP Part A licensed pharmacist in good standingOCP Injection Certification – current certification and first aidCertified Diabetes Educator (CDE)
