Our Nursing Team works with community and hospital partners to provide excellent collaborative care, focusing on social determinants of health. This role performs a variety of functions including administration of psychotropic injections, health teaching, medication reconciliation, coordinating appointments, providing diabetes care and lifestyle supports to afford clients the ability to achieve success with their recovery goals.Within the Housing Department, the RN will support residents living in congregate as well as independent and youth housing environments. While working within Medical Services, the RN will support various community physician and psychiatric clinics. Hours differ in both departments.WHAT WE'RE OFFERING:What you'll be making -Starting rate for this role is $32.13 per hour (Level 1 – RN/OT).Successful external applicants will start at Level 1, with grid progression based on seniority up to Level 8 ($41.41 per hour); for internal applicants, grid level placement will occur as per the Collective Agreement.When you'll work– 70 hours over two-week period; Monday to Friday,8:30am-3:30pmYour pension plan– Immediate enrollment in the Healthcare of Ontario Pension Plan (HOOPP)What your benefits will be –After a 90-day waiting period, will be enrolled in the extended healthcare benefits planHOW YOU WILL BE MAKING A DIFFERENCE:Nursing Assessments:Conducts physical and psychiatric assessments and makes appropriate referrals to community physicians and resourcesEngages in direct care, mentoring, education, consultationAssesses medication effectiveness, complications and side effectsArranges and follows up with required lab workProvides symptom education to enable participants to identify their mental illness symptomsCoordinates mental health and psycho-social serviceIdentifies and develops and maintains strong links with a wide range of community partners/resourcesCoordinates and administers Depot Injections to participantsDevelop and facilitate groupsHealth Promotion:Assists clients in making informed choices about health measures such as medications, immunization,



palliative care, birth control, disease management, etc.Provides health teaching by giving feedback, explanations and rationale for diagnostic tests, procedures and treatmentsProvides teaching and research, as requiredAssists clients in taking responsibility for and managing their health needs according to available resources and personal skillsIdentifies and seeks to address the root causes of illness and diseaseAssists clients in making healthier choicesEstablishes robust links to the inpatient team at LHSC for referrals to outreach injection programCollaborates with community agencies to maintain coordination in the treatment processAdvocates for clients to obtain psychiatrists, family doctors, dentists, etc.Direct Service:Identifies individual care requirements by establishing personal rapport with clients and with other persons in a position to understand care requirementsAssures quality of care by adhering to therapeutic standards; measuring health outcomes against goals and standards; making or recommending necessary adjustments; following hospital and care providers’ philosophies and standards of care set out by the College of Nurses standards and other governing agency regulationsProtects clients and employees by adhering to infection-control policies and protocols, medication administration and storage procedures, and controlled substance regulationsMaintains continuity among nursing team by documenting and communicating actions, irregularities, and continuing needsMaintains nursing supplies inventory which may include checking stock to determine inventory level; anticipating needs supplies; placing and expediting orders for supplies; verifying receiptof supplies; using equipment and supplies as needed to accomplish job resultsSpecific to Congregate Housing environments:Assist residents to maintain good personal hygiene which may include but is not limited to bathing, dressing and continence care.Assist residents with elimination and toiletingSupporting individuals with transfers and personal care.WHAT YOU'LL NEED TO APPLY:Police Vulnerable Sector Check Requirements:Successful candidates must have a Police Vulnerable Sector Check completed in the last six months.
