POSITION SUMMARYThe role of the Sherwood Park – Strathcona County PCN Family Practice Nurse is to provide comprehensive nursing services to the patient population of a family practice clinic within the Network. The nurse will work collaboratively with patients, family physicians and other team members to develop and implement care plans and provide follow-up with patients. The nurse will assess, manage and support patients to provide comprehensive care, education and guidance to patients with a variety of clinical conditions.
