Staff are onsite, 24 hours a day, 7 days per week, and provide supervision and support in a safe, stable environment to help residents gradually readjust to the community after living in a secure hospital setting. This Program is delivered in partnership with the FACS NCR Treatment Team.TheCommunity Liaison/Recreation Workerwill help clients safely and gradually readjust to the community after living in a secure hospital setting. They are also responsible for the day‑to‑day operation of the Home, teaching basic living skills and facilitating community activities and outings.ResponsibilitiesGoal setting with the clients that are specific to risk factors and compliment their special conditions of releaseDelivery of services in a client-centred, strength-based approachCompletion of accurate written documentation, data collection/entry, intakes and assessmentsContribute to the management of high risk,



high needs client base by being a member of multidisciplinary teamEnsure services are being delivered in accordance with Agency Policies and ProceduresContribute to a positive culture by assisting and supporting colleagues as needed, and encourage and teach others about the unit’s operationsMaintain a safe, secure and clean environmentParticipate in Unit Meetings, Agency Meetings, Case ConferencesQualifications / SkillsDegree or Diploma in a human services field with related experienceValid First Aid/CPRValid Class 5 Driver’s License (No GDL)Solid communication and pro-social relationship building skillsDemonstrated ability to communicate effectively with individuals with mental health conditionsKnowledge of the mental health populationExperience working with a multidisciplinary team environmentKnowledge of local activities and events is an assetA Criminal records check is required.
