Position SummaryThe Community-Based Service Coordinator supports First Nations and Inuit children, youth, and families in accessing services and supports through Jordan’s Principle and the Inuit Child First Initiative.This role works directly with families, service providers, schools, health professionals, and government partners to help coordinate access to health, social, and educational supports. The Coordinator plays a key role in reducing barriers and ensuring children receive timely, culturally safe, and appropriate services.This position is part of a broader provincial network of Community-Based and Regional Service Coordinators working collaboratively across British Columbia.Full-Time (35 hours per week)Term position until March 31, 2027- with continuation subject to funding renewal$35.00 per hourFull benefits provided through GNN, after successful three month probation periodCombination of in-office and remote workKey ResponsibilitiesIntake, Assessment & CoordinationServe as a primary point of contact for First Nations and Inuit children, youth, and familiesSupport submission of Jordan’s Principle and Inuit Child First Initiative requestsAssist with intake, assessment, and coordination of servicesIdentify immediate safety or risk concerns and respond according to policy and legislationMaintain continuity of care through accurate documentation and follow-upCase ManagementDevelop and document coordinated service plans for children and familiesSupport access to approved health, social, and educational servicesAssist families with navigating funding approvals and service systemsEnsure follow-up to confirm services are delivered and needs are metMaintain accurate and timely case recordsOutreach & Community EngagementBuild and maintain relationships with schools, health providers, and community organizationsCollaborate with service providers including health professionals, educators,



and social service agenciesWork with Indigenous and non-Indigenous partners to support service coordinationPromote awareness of Jordan’s Principle and Inuit Child First Initiative supportsProvide regular updates and reporting to partners and leadershipCollaboration & Service CoordinationWork closely with Regional Service Coordinators and the BC Jordan’s Principle HubSupport communication between families and Indigenous Services Canada (ISC)Respect existing service relationships within communitiesShare best practices and support coordinated service deliveryAppeals & Advocacy SupportAssist families in preparing and submitting appeals when requiredSupport navigation of federal and provincial systemsAct as liaison between families and ISC when neededQualificationsBachelor’s degree in Social Work, Health, Education, or related fieldMinimum 3 years’ experience in social services, health, education, or case managementStrong knowledge of Indigenous communities, cultures, and contemporary issuesExperience working with vulnerable populations and family support systemsStrong communication skills (oral and written)Solid organizational and case management skillsAbility to work with multiple stakeholders (families, schools, government, health systems)Proficiency in Microsoft Office (Word, Excel, email systems)Valid BC Driver’s License and reliable vehicle requiredSuccessful Vulnerable Sector Criminal Record Check requiredOR relevant diploma or certificate in a related human services field may be consideredAssets (Preferred)Experience with Jordan’s Principle or Indigenous child and family servicesKnowledge of BC health and social service systemsKnowledge of Indigenous languagesExperience in community-based outreach or coordination rolesAdditional RequirementsStrong time management and ability to work independentlyAbility to manage sensitive and confidential informationUnderstanding of Indigenous cultural safety and trauma-informed practiceEquity StatementPursuant to Section 41 of the BC Human Rights Code, preference may be given to qualified applicants of Indigenous ancestry.#J-18808-Ljbffr
