The successful candidate will play a critical role in supporting adults during their transition from hospital care to community-based services, ensuring a smooth and effective discharge process.What will your responsibilities be?Conduct comprehensive assessments of patients' needs and develop individualized discharge plans in collaboration with healthcare professionals, patients, and their families.Manage a caseload of patients, ensuring timely and coordinated discharge from the hospital to appropriate community or residential settings.
