Click here to apply as an internal applicant.Req ID: 216028Location: Western Zone, Annapolis Community Health CentreDepartment: PH Enhanced Home VisitingType of Employment: Permanent Hourly FT (100%) x 1Posting Closing Date: 23-Feb-26About The OpportunityReporting to the Public Health Early Years Manager, the Community Home Visitor (CHV) works in collaboration with the Early Years Consultant and others as appropriate to ensure Early Years programs and services are planned and implemented to address local population health needs.The CHV works with at‑risk families and vulnerable populations during the prenatal, postpartum, and early childhood period. The CHV implements the Healthy Beginnings Enhanced Home Visiting program including strengths‑based curriculum for at‑risk families. The CHV is part of a collaborative multidisciplinary team approach and works with the Early Years Team to ensure quality case management. The CHV shares a core set of public health attitudes and values, applies public health core competencies to all the work and is accountable to provide competent, safe and ethical professional practice. The CHV ensures that the principles and practices that guide Nova Scotia Health such as the vision, mission, values, strategic directions, ethics, safety, quality, partnerships and inter‑professional collaboration are integrated within the work that they do and through the messages delivered.About YouDiploma in Early Childhood Education or Human Services from an accredited education institution requiredA valid driver's license and access to a reliable vehicle, travel will be requiredCultural First Nation Competency Training an assetCurrent CPR and First Aid CertificationDemonstrates the core competencies for public health as outlined in the job analysisDemonstrates knowledge in the Health Protection Act and other relevant Regulations, Legislation & Acts,
