JOB SUMMARY Reporting to the Navigation & Education Manager, you will serve as a trusted point of contact for people living with dementia and their care partners following a diagnosis. You will identify individual and family needs, provide timely information and education, help clients establish practical self-management goals and support plans, and connect them with appropriate health and community services.

By strengthening communication, coordination and care planning across providers and sectors, you will help improve the client experience, continuity of care and system capacity. Your work will build care-partner knowledge and confidence, reduce the risk of crises,



and support people living with dementia to live safely in their homes and communities for as long as possible.

This role is ideal for a compassionate bilingual professional who enjoys relationship-based work, navigating complex systems and making a meaningful difference in the lives of people on the dementia journey.

WHAT YOU WILL DO

Initial Support and Care Planning — 35%

- Respond promptly to First Link referrals and provide early support.
- Meet with people living with dementia and their care partners to understand their strengths, needs, goals and risks.
- Complete or review assessments and work with families to develop personalized care and support plans.
- Identify the services and professionals involved and help clarify everyone’s responsibilities.

Service Navigation and Care Coordination — 35%

- Help families understand their options and connect them with appropriate services, education and community resources.
- Coordinate communication and care planning among families, physicians, healthcare professionals and community service providers.
- Arrange care meetings when needed and work with partners to address barriers to service.
- Build relationships with health and community providers and increase awareness of First Link.

Ongoing Follow-Up and Care Transitions — 20%

- Check in regularly to identify changing needs and ensure services continue to work together.
- Support people living with dementia and their care partners through transitions between health, social and residential care services.

Records and Program Improvement — 10%

- Maintain accurate and timely client records while following privacy requirements.
- Track and report program information and participate in program evaluation.
- Help improve First Link resources, processes and services.

QUALIFICATIONS





- Education in nursing, health sciences, social work, gerontology or a related field, with current registration with a regulatory college where applicable.
- Three to five years of relevant client-service experience in health and/or social services.
- Experience supporting people living with Alzheimer's disease or another dementia and their care partners.
- Experience in assessment, care planning and coordination, including support for chronic or complex health conditions and knowledge of community care options.
- Strong communication, facilitation, shared decision-making, workload-management and technology skills.
- Demonstrated cultural sensitivity and the ability to work independently and collaboratively.
- Skilled proficiency in spoken and written English and French is required.

ADDITIONAL ASSETS

- Knowledge of person-centred dementia practices, including P.I.E.C.E.S.
- Understanding of primary care, community care and specialized geriatric services.
- Experience working in an interprofessional, collaborative environment.

WORKING CONDITIONS AND TRAVEL This position includes general office duties, regular computer use and travel throughout the chapter district. A valid Ontario driver's licence and access to a reliable, insured vehicle are required.
