About The Program: Assisted Living – College View is a partnership with Toronto Seniors Housing Co. providing 24/7 assisted living services to vulnerable older adults and seniors living with complex needs.



The program serves over 100 clients in the Downtown East Toronto OHT. Assisted Living – College View serving 75 residents in the building, while also providing services to clients in the wider community, primarily by referral through the Crisis Outreach Services for Seniors program. The program works with each client on an individualized client care plan based on their wants, needs, and strengths. Moreover, LOFT staff utilize a standardized assessment tool called the RAICHA to highlight and mitigate risk for decline in the client’s health. LOFT case managers work with clients to identify and mitigate barriers that affect their physical and mental health, integrating social determinants of health into their practice.
