Intensive Case CoordinatorReporting directly to the Sohkayawin Housing Manager, the Intensive Case Coordinator delivers housing stabilization and intensive case management services to tenants within the apartment complex. The role involves working in a high‑risk environment with individuals experiencing complex challenges, including mental health concerns, substance use, and histories of trauma. The Coordinator provides a holistic, trauma‑informed, and culturally responsive approach to care, coordinating team efforts and ensuring tenants maintain reliable housing and improved overall wellbeing.ResponsibilitiesProvide intensive, tenant‑centered case management to individuals living in permanent supportive housing.Develop, implement, and regularly update individualized case plans focused on housing stability, health, and personal goals.Conduct daily tenant house checks.Support all aspects of tenant move‑ins and transitions, including unit setup, welcome supports, and orientation to the building.Assist tenants in maintaining housing, including cleanliness, guest management, and adherence to tenancy agreements.Assist tenants in accessing income supports, identification, and essential services.Coordinate and support access to healthcare, mental health services,
addiction supports, and cultural programming.Provide transportation and/or accompaniment to critical appointments, advocating on behalf of tenants when needed.Support crisis intervention and de‑escalation, utilizing trauma‑informed and harm‑reduction approaches.Foster positive tenant‑landlord/property management relationships and support conflict resolution when necessary.Facilitate life‑skills development (budgeting, cooking, cleaning, medication routines, daily living skills).Collaborate with internal team members and external agencies to ensure wraparound supports.QualificationsUniversity bachelor’s degree.3–5 years of experience in case management or related support services.Compensation$27 – $30 per hour, based on education and experience.Employment DetailsFull‑time, with employment until March 31 2027 and a possibility of extension.
