Many encounters are handledvirtually, within-person visits required only when clinically necessary and pre-approved .Each Nurse Practitioner is assignedone service area, with coverage structured to be focused, predictable, and collaborative.Each Nurse Practitioner supports asmall cluster of neighboring communitieswithin Ontario Health at Home’sCentral East service area .Examples include:Durham Region (Pickering, Ajax, Whitby, Oshawa)HaliburtonPeterboroughKawarthaThe role isvirtual-first,
