Enhance community health as the Home and Community Care (HCC) Public Health Lead in Thunder Bay, Ontario. Drive public health initiatives while providing nursing services and education in various KO communities. Reporting to the HCC Nurse Manager, this role involves developing and implementing the HCC/PH program to meet clients' diverse care needs.
You will work closely with Personal Support Workers and deliver essential public health programs. The position focuses on client-centered care, ensuring adherence to nursing standards and organizational values. Key Responsibilities:
- Assess and evaluate nursing programs focused on public health
- Act as a resource for communities and HCC teams
- Provide health education and advocate for policies
- Participate in community coalitions and health planning
- Oversee Personal Support Workers and educate the HCC team Requirements:
- Registered Nurse certification with Ontario's College of Nurses
- Minimum 2 years of clinical practice experience
- CPR/BLS certification
- Advanced Foot Care certification or willingness to obtain
- Ability to communicate in Cree or Oji-Cree is an asset Lead public health initiatives and support community wellness through nursing expertise in Thunder Bay.
