Join our D2 unit as a Registered Practical Nurse, committed to person-centred, evidence-based care for internal medicine and nephrology patients. Collaborate within an interprofessional team to enhance patient outcomes. This role emphasizes independent and team-oriented nursing practices, providing a patient-focused approach in a fast-paced, acute care setting.
You'll engage deeply in care assessment, planning, and evaluation while supporting continuous career development and quality improvement. Key Responsibilities:
Assess and identify patient and family needs thoroughly
Plan and reassess care strategies based on evolving needs
Implement and evaluate evidence-based interventions
Foster a culture of quality improvement in nursing practice
Work closely with RNs to ensure comprehensive care delivery Requirements:
Active registration with the College of Nurses of Ontario
Current certification in CPR from the Heart and Stroke Foundation
Preferred diploma in RPN with acute care experience
Completion of Sunnybrook Advanced Assessment Course within 2 years
